Slay Transportation has changed their log form to J. J. Keller's form 0803 with their company information pre-printed on that form. Is this form already available or do I need to send 2 copies (originals) to be scanned into your system. If it is already available, how do I change to the new form.

I'm sorry, I don't have their new log form. You can send me 2 -3 of your log forms with the company name and address preprinted on them and I can add it to your account. Please DO NOT fold the logs when you send them in, it messes up the scanning. Also include a note with your name and number so I know which account to add it to. I do not have just a generic JJ Keller log form on file. Does your company give you preprinted logs with the company name and address on them?
