Picking A Form

Our most popular form ships with the Eclipse Logbook software and use it immediately. If you need or want a different form, we can often help. Find out what information you need (below) and then Contact Us.

For Paper Logs Pre-Imprinted with Carrier Info

Let us know the carrier name and address. If we have the form on file we can attach it to your account in one business-day or less in most cases.

If we do not yet have the form in our library, AND your carrier will confirm to us they can't accept one of our standard forms, we can usually generate their exact form at no charge. Contact us to make arrangments and to get an estimate for programming time. Due to copyright restrictions we must include the carrier name and address on carrier-specific forms; we can not "blank out" that information.

For Paper Logs Purchased at Truck Stops, etc.

If you buy or use forms with NO pre-printed carrier name, address, etc., and you don't like the standard form that comes with Eclipse Logbook, we have alternative standard forms that are available quickly and at no charge.

Let us know what you don't like about our existing forms, and we'll suggest an alternative standard form. Additionally we can sometimes make new forms (at no charge) if we think they will be popular with other drivers.

We are often unable to provide exact duplicates of forms purchased at truck-stops due to copyright restrictions by the form's publisher; therefore using one of our alternative forms is usually the best choice.
